Fantastic news – the NHS C&M submission in the Neighbourhood Health category has been named regional champion in the NHS Excellence Awards. The winning submission showcased the work being led through the Liverpool Place Population Health Group, highlighting a wide range of collaborative activity across the system. This included the proactive use of CIPHA (secure data platform) within Integrated Care Teams, alongside Telehealth and other specialist services delivered by partner organisations.
This is a brilliant achievement and a strong endorsement of the population health management approach being driven through Liverpool Place – with Mersey Care a very active partner in this work. It’s also encouraging to see that similar groups have now been established across the other places within Cheshire and Merseyside, reflecting the wider impact and scalability of this approach.
