Safe Management of Controlled Drugs Audits
- In -patients and some community clinics (including Dental services and Wic/UTC) quarterly audits carried out by the MMT. Findings and action plans are reported via AMaT
- The audit tool is displayed on the teams Amat dashboard as MEDICINES MANAGEMENT TEAM ONLY-Controlled drug audit
- The management of lower schedule drugs is also completed by MMT at the request of the CDAO, MMT Schedule 4 part 1 and 5 controlled drugs
- Nursing leads will carry out additional weekly audiots for added assurance. This is a temporary measure and its need will be reviewed regularly.
End of Life Controlled Drugs Audits
- Safe destruction of Controlled drugs audited in line with the new CD procedure . This audit is completed by individual teams managing controlled drugs in patient's own homes. The audit tool will be displayed on the teams Amat dashboard
Antimicrobial Prescribing Audits
- In patients -Quarterly audits undertaken by MMT. Currently carried out on MS forms
- Community- Quarterly audits undertaken by individual teams . the audit tool will be displayed on the individual teams Amat dashboard.
Safe and Secure Medicines Audits
- At least annual audits carried out by the MMT. The audit tool will be displayed on the teams dashboard indicating it will be undertaken by the MMT.
- Quarterly spot checks will be carried out by individual teams .The audit tool will be displayed on the teams Amat dashboard as Quarterly Team Safe and Secure medicines audit
- Findings and action plans are reported via AMaT

- Currently being piloted for teams holding a shared prescription pad. (Sept 25) Further information will be disseminated once the audit is live.

The Prescribing Observatory for Mental Health (POMH) is a subscription based project that helps specialist mental health services across the UK improve their prescribing practice.
To achieve this, POMH develop audit-based Quality Improvement P rogrammes (QIPs) that focus on specific topics within mental health prescribing.
The Trust participate in POMH audits which are led by the medical team with oversight from the Medicines Management Team and the audit team.
