- Nurse triage team (UCT) consisting of only 4 WTE resulting in poor resilience and a reliance on District Nursing staff to plug gaps or employ bank staff to cover gaps in rota
- SPOA staff covering multiple community nursing services.
Liverpool and Sefton
- SPC operated 8am to 5pm Monday to Friday – now aligned with Knowsley hours following separate consultation
- Ongoing recruitment to Nurse Advisor roles in SPC.
Proposal
- To streamline the District Nursing access for patient and referrers into one delivery model via one team
- Unplanned Care Team in Knowsley (4 WTE) to move into the Nurse Advisor team into SPC team
- During the transition process, there may be a requirement for Knowsley Nursing Services to continue providing bank support to cover the Knowsley specific triage process – all staff will receive appropriate training to undertake this
- SPOA Band 3 admin staffing resource (8 WTE) to move into SPC team
- SPOA Band 4 admin lead (1 WTE) to move to SPC team
- Provide clear referral pathways and access into District Nursing services, 7 days per week, 8am to 8pm
- Step further towards single SPC across Community Division.
