The Tissue Viability Nurses are arranging two further sessions on how to implement PURPOSE-T (Pressure Ulcer Risk Primary or Secondary Evaluation Tool), which is available to all staff across the Trust. Tissue Viability Nurses will be discussing the changes and the roll out of the new pressure ulcer categories which involve the removal of the term ‘Deep Tissue Injury’ (DTI) and ‘unstageable pressure ulcer’. It is important that all clinical teams are aware of these changes in categorisation and reporting. RADAR has also been updated.
As identified in the new Trust wide Pressure Ulcer Policy (SD56) the Waterlow risk assessment was replaced with PURPOSE-T on 31 August 2024. If the Waterlow template is still appearing for some staff on EMIS, please log a job with the IT department to have the Waterlow template removed.
