A patient safety incident has been identified where clinical information and discharge communications have not been successfully transmitted to GP practices in South Sefton via EMIS.

Whilst this incident has been isolated to South Sefton GPs, it is essential that all teams using EMIS web send documents following the correct process.

To prevent delays in clinical information being received there are actions that need to be in place with immediate effect.
