There has recently been an updated Verification of Expected Death policy and template shared across Community Care division for all Mersey Care staff that should now be in use for patients who pass away whilst under the care of a Mersey Care team. The template (Appendix 1) in the Verification of Expected Death policy has now been uploaded onto EMIS for paper lite use however two additional boxes have been added to this to capture information pertinent to patient care at End of Life in line with the VOED policy. These boxes are as follows:

  • The parental infusion was calculated and confirmed as delivering the correct amount
  • The syringe driver contents have been denatured according to the VOED Policy.      

As a part of the Verification of Expected Death policy, these should be checked by the registered professionals present and ticked on the EMIS VOED template should the information be correct prior to signing or saving the document.

If there are concerns with the content of the syringe driver, a GP should be contacted to attend to conduct the verification of death and the concerns should be escalated according to escalation procedures in your team alongside of the completion of a RADAR to further review the issues noted.

The template on EMIS can then be sent to the GP via Email, EMIS, or fax in line with the VOED policy in timely manner to ensure continuity of care is provided for the service user, family, friends or carers. The new VOED template will replace the original template on EMIS to prevent incorrect documentation from being completed from next week.
