A huge thank you to those colleagues who kindly took part in our survey and provided feedback on proposed name change for the Community Assessment Team and Mersey Care Assessment Team. We will be back in touch in the coming weeks once the survey is closed down and the new name for the team has been approved.
In the interim, please see attached the updated NHS Continuing Healthcare referral form for immediate use and the details of the Mersey Care Assessment Team (temporary name).
