From the 1 April the Medicines Management Liverpool GP Support and Care Home Team will be leaving Mersey Care to join Cheshire and Merseyside ICB, forming part of what will be an ICB wide medicines management service.
The team have worked side-by-side with Liverpool based community nursing and AHP colleagues for approaching 20 years, working across the interface with primary care.
During this time, they have supported many of the most vulnerable community based patients across Liverpool to get the best outcomes from their medicines through structured medication reviews, and have been an invaluable source of advice and support around medicines for colleagues within the Community Care Division.
The team have developed fantastic and mutually beneficial working relationships with community colleagues such as the Community Matrons, DNs, ACPs, Medics and AHPs. They have supported the implementation of, and been critical in the success of, workstreams such as the Enhanced Health in Care Homes work, ICT Multidisciplinary Review Pathway, the Intermediate Care Hubs and winter pressures work.
We will continue with the professional links with the team following transfer to ensure optimal patient care and as a system continue to provide a quality medicines management service.
