The Remote Monitoring enabled Heart Failure Ward was announced Winners at the HSJ Patient Safety Awards in the category of Safety Improvement Through Technology.
Mersey Care’s Telehealth Team provides the monitoring for patients accessing this service who are also supported by some of our Community Nursing Services. Supporting over 300 patients since going live in May 2022, the pathway has demonstrated a 50 per cent reduction in 30-day readmission rates, along with a 50 per cent reduction in 30 day mortality.
