The Pathology team need, as a minimum, the same three patient identifiers on both tubes, as well as the request
forms in order to be processed. Without these, the sample will be rejected and not analysed.
They require 1,2 & 3 or 1,2 & 4 as minimum patient identifiers:
1. First and last name,
2. Date of birth,
3. NHS or hospital number
4. First line of address if NHS/Hosp number not available.
Please make sure you do this to avoid delays and remove risk to patient’s management.
