Mersey Care recognised the need for improved mental health services for children, young adults and young adults transitioning to adulthood in our region. The goal of the new process is to provide a seamless experience for patients. The focus is on ensuring continuity of care, involving patients in decision making and supporting their wellbeing as they navigate the transition from children's to adult mental health services.
There are risks and challenges associated with this transition and we understand the importance of a co-ordinated and person centred approach to support young adults in their mental health journey.
Service users and referrers should see no change to pathways and treatment but will experience an improved service as they transition into adulthood.
The transition to adulthood policy has been reviewed and updated, in line with national guidance that outlines the principles that support safety and quality. A suite of interventions has come out of this which will be shared with staff and young people to help them make informed decisions about their care.
A number of 18 to 25 test sites (Early Intervention in Psychosis Teams) are benchmarking against 18 to 25 standards and reporting into an 18 to 25 operational and quality group, overseeing performance, risks and quality of the pathway.
