Waiting list management
Waiting list management is linked to many areas of the community excellence transformation programme. Our new community model is designed to identify patient need and offer intervention based care. There will be patients who need to be on the caseload for an extended period, however, there will also be those who present with a need that can be supported with a shorter term intervention or through our Step Forward service. The community excellence framework also discusses the option of care being provided by organisations within the community by voluntary, community, faith and social enterprise (VCFSE).
This new way of working with easy access to services and seamless step up and step down processes, ensures that static caseloads will be more manageable for teams.
To achieve this, caseload management work is taking place with our teams to review whether service users on our current case load are receiving the intervention that best suits their needs. The transformation team is supporting community teams during this process.
