As part of our continued commitment to improve our digital infrastructure, the Southport and Formby Falls service will be moving from EMIS Org 141137 from 24 November 2025.
The Falls service will migrate onto our existing ‘Therapy Services’ EMIS Web org CBD 24540. The migration aims to enhance the integration and coordination of therapy services, providing a more holistic approach to patient care.
The current referral form will continue to be used for Southport and Formby Falls, and will be sent via email to Single Point of Access (SPOA) southportandformby
Using ‘All records’ in EMIS Web will continue to provide access to the full community shared record which will become more enriched as services migrate. Please Note: this will be for Southport and Formby GPs who have agreed to Data Sharing only.
This is a planned migration of EMIS across therapy services within Southport and Formby; the remaining teams are in the process of planning their go live date and you will be updated with further information when this happens.
