Our Telehealth Remote Monitoring System is moving from Docobo to Graphnet Remote Monitoring (powered by Luscii).
This change is part of Mersey Care’s wider digital transformation programme to improve remote monitoring and support better patient outcomes.
Graphnet Remote Monitoring (powered by Luscii) will become the standard remote monitoring platform across Cheshire and Merseyside.
Health Care Professionals
- Continue to refer patients for Telehealth remote monitoring as usual
- Referral processes, escalation pathways and clinical responsibilities remain the same
- No changes to long term condition monitoring or specialist pathways.
For Patients
- Existing patients will move to the new system by 3 March 2026
- Regular patient communications will be sent throughout the transition
- On the day they move, patients will receive a text message or email guiding them to download the Luscii app
- Telehealth staff will monitor both systems during the dual‑running period (23 February to 9 March 2026) to ensure safe, uninterrupted care
- Patients are reassured that Luscii is an NHS‑supported system, and support is available if needed.
