0 – Low – This person has no history of self injury.  There is no reason to believe this person will engage in serious injurious behaviour.

1 – Low – This person has a history of minor self injury.  They have not displayed any such behaviour or expressed a desire to self-injure within the last 12 months.

2 – Low – This person has a history of minor self injury.  They have not displayed self-injurious behaviours in the past 12 but have expressed a desire to self injure in the last 12 months.

3 – Medium – This person has a history of minor self-injury and has harmed themselves within the last 12 months.

4 – Medium – This person has a history of self-injury they harm themselves to the extent of requiring limited medical attention e.g. bandaging or sticking plaster.

5 – High – This person frequently self-harms in a manner that requires limited specialist medical attention e.g. the wound may need stitching, non-urgent surgery, x-ray.

6 – High – This person frequently self-harms in a manner that requires extended specialist medical attention that has required admission to hospital or to the hospital ward. 

7 - High – This person frequently self-injuries to the extent that there is a distinct probability that loss of function of life might follow.

4- Medium – This person has a history of Para-suicide, but not in the past year.  They have said that they might kill themselves or be better off dead in the past 2 months but have not attempted to harm themselves.

5 – High – This person may or may not have a history of Para-suicide.  They have stated that they might kill themselves or be better off dead in the past month.  These are other factors that might suggest they would make an attempt on their life.  For example, bereavement or mental illness. 

6 – High – This person may have a history of Para-suicide they have made half hearted attempts to kill themselves in the past 3 months.  There are other factors that might suggest they would make an attempt on their life.  For example, bereavement, mental illness.
