Pre-referral form – supporting a good wellbeing culture and Occupational Health referrals
This information is to support the user testing of our new pre-submission form for Occupational Health referrals, via OPAS G2.
Why pilot a pre-referral form?
The pre-referral form's aim is to better support both managers and their teams.
Occupational Health referrals can make a real difference to your team's health and wellbeing, but it's important that referrals are well informed and reflect your team member's needs.
Working with Civica (OPAS G2) we've created a process to help you identify if an Occupational Health (OH) referral is necessary, or if there is an alternative solution which might be more appopriate.
How do I use the pre-referral form
When you log into OPAS G2 (Civica Occupational Health) to create a new management referral, you will automatically access the pre-submission questionnaire.
Following an initial conversation with your team member, walking through the questions will help you to determine if a management referral to Occupational Health is required.
Answering yes to any of the 7 questions, allows you to see the support on offer, with signposting available to you and your team member.
If you no longer feel a referral to OH is required as you have the support available to meet your team member’s needs, click the red box 'Complete Referral OH not required' and then ‘confirm’.
After following the steps in the pre-submission form, if you still feel a referral is required, click the green box 'Continue Referral Process' , and then ‘confirm’.

To understand if the pre-referral form is working, we need your feedback as part of the pilot running until July 2025.
Managers who test the pre-referral form will receive a notification asking for your feedback and suggestions, before the pre-referral form goes live.
