As part of our continued commitment to improve our digital infrastructure, Planned Therapy Sefton and ICRAS North Sefton will both be moving from EMIS Orgs 141137 from 28 May 2025.
Planned Therapy Team will migrate onto the existing Mersey Care ‘Therapy Services’ EMIS Web org CBD 24540. The migration aims to enhance the integration and coordination of therapy services, providing a more holistic approach to patient care.
Referrals to the Planned Therapy Team, including both Occupational Therapy (OT) and Physiotherapy (PT), will continue to use the current referral form and will be via email to Single Point of Access (SPOA) southportandformby
ICRAS/UCR will migrate onto the existing Mersey Care ‘Unplanned Care’ EMIS Web org CBD 23446. This migration will also enhance the integration and coordination of UCR services across Mersey Care.
Any patients who are at risk of a hospital admission and require an urgent therapy assessment as well as any frailty referrals, must call UCR via Single Point of Contact (SPC) on 0300 323 0240 and select option1.
If ICRAS referrals are sent to Planned Therapy Sefton, these will be declined at point of triage and the referrer will be notified to call SPC directly.
Using ‘All records’ in EMIS Web will continue to provide access to the full community shared record which will become more enriched as services migrate. Please Note: this will be for Southport and Formby GPs who have agreed to Data Sharing only.
This is a planned migration of EMIS across both nursing and therapy services within Southport and Formby, the remaining teams are in the process of planning their go live date and you will be updated with further information when this happens.
