The North Mersey Integrated Provider Partnership Lung Health Programme brings together Alder Hey, Mersey Care and University Hospitals of Liverpool Group to improve lung health across Liverpool, Sefton and Knowsley. The programme focuses on collaboration between clinical, operational and corporate teams, alongside partners including General Practice, primary care, neighbourhoods and the communities we serve.

Lung health is the first integration focus because all three organisations already deliver community‑based lung health care.  This provides a strong foundation for improvement and shared learning. While many strong respiratory services already in place, they have often developed separately, leading to variation in access, pathways and patient experience.

For our staff, this programme is about working differently across organisational boundaries to deliver more consistent, preventative and joined‑up lung health care from childhood through to older age.

 

What are we trying to achieve?

We want to move from a system that mainly reacts to illness, to one delivers practical, scalable improvements by:

  • Identifying risk earlier and preventing avoidable deterioration
  • Providing seamless care across childhood and adulthood
  • Connecting hospital, community and primary care more effectively
  • Supporting families as well as individuals
  • Using data to help reduce health inequalities
  • Actively involving patients, carers and staff in shaping services
  • Shared workforce models, joint training and rotational opportunities
  • Improved digital and information sharing.
