Join the first Trust-wide Learning from Excellence Forum

The first Trust-wide Learning from Excellence Forum will be held on 4 July 2025 from 10am to 12.30pm.  This face to face forum is to allow wider sharing of examples where patient safety or experience have been improved as a result of feedback from incidents or patient stories.

This is open to practitioners from any division, however numbers are limited at present. Please send your request to join to PSIRFenquiries@merseycare.nhs.uk . If the forum is over subscribed we will hold your name on a reserve list in case of cancellations.

We are also still seeking a small number of presenters. If you have real life examples of either patient stories that have led to changes in care processes or patient safety incidents which has underpinned improvement work that can be shared, please get in touch with kim.bennett@merseycare.nhs.uk ’
