FP10 Pads for South Sefton NMPs

From 2 April, all prescription pads (FP10s and outpatient prescriptions) for South Sefton NMPs will move to The Medicines Management building at Maghull Health Park. Last date of collection for prescription pads from the NMP team at V7 is Friday 22 March 2024. 

Any request for prescription pads made on or after 2 April will now be in line with Trust Wide procedure and order forms can be found on YourSpace once you access this page the requests forms can be found at the bottom of the page. Please allow five working days for new requests to be ordered and ready for collection.

Please continue to contact Terri.powell@merseycare.nhs.uk to book an appointment to collect prescriptions up until this date.

Self declarations for Trust Wide NMPs

In addition to the above change please note as of 26 March 2024 all Trust wide NMP self-declarations will be managed by the NMP team at V7. This includes annual declarations, name, address and team changes.
