Work is ongoing to align divisional medicines-related policies and procedures to create Trust wide documents that suit all services.
All aligned Trust wide procedures will take the prefix ‘MMP’. The most frequently used procedures will be assigned MMP01-10. All MMP procedures are to be read in conjunction with the SD12 Safe and Secure Medicines Policy.
The following Trust Wide medicines management owned procedures have now been approved and uploaded to YourSpace:
- MMP11 Discharge Medicines Service (DMS)
- MMP12 Use of Buprenorphine Prolonged-Release Injection (Buvidal®)
- MMP13 Non Medical Prescribing within the Anticoagulation Service

MMP11 Discharge Medicines Service (DMS)

- Applicable to all inpatient wards
- New procedure
- DMS enables NHS Trusts to send referrals direct to the nominated community pharmacy of patients who are eligible and would benefit from extra guidance around newly prescribed medicines. The referral includes a copy of the patient’s discharge prescription. The aim of the service is to reduce readmissions to hospital.
- The automated referral process and eligibility are determined in this procedure.
- This procedure supersedes the Mid Mersey Transfer of Care Around Medicines (TCAM) procedure. TCAM was replaced by the DMS last year.
- DMS is live in Mid Mersey Division and will be implemented in all other divisions.
MMP12 Use of Buprenorphine Prolonged-Release Injection (Buvidal®)
- Applicable to Addictions Services only
- New procedure to support the pilot of Buvidal® clinics
- This will be reviewed in 12 months
MMP13 Non Medical Prescribing within the Anticoagulation Service
- Applicable to the Anticoagulation Service only
- The procedure was introduced 12 months ago with minimal changes made during the review
- 3 year review date assigned
