Medicine Safety Week is about making medicines and medicine devices safer. The theme this year is around the importance of taking medicines in the right way to prevent side effects and to encourage reporting of side effects when they occur.
This can be done via the Yellow Card Scheme which is where side effects are reported and promoting this across the Trust.
Mersey Care believes strongly in setting the highest of healthcare standards across everything we do. This includes zero tolerance of harm from medication, which we aim to achieve through the following steps:
- Zero moderate / severe harm incidents relating to medicines
- Zero harm from prescribing, administration and management of controlled drugs
- Zero harm from Valproate
- Zero medication administration errors linked to patient misidentification.
