Our teams are supporting preparation for our go-live with Learning from Patient Safety Events (LFPSE) reporting to NHS England, direct from our Radar incident and risk management system, from 30 May 2024.

The Radar user guides (slides and demo videos) will be uploaded to YourSpace by the 20 May along with additional details about LFPSE information collection.

A significant amount of work has taken place to make sure the system is ready to report as required. Please see below some frequently asked questions and their responses from our subject matter experts:

A. All comments raised in the clinical division sessions have been addressed/considered. However, some comments related to the LFPSE questions are outside of our influence and are mandated (for all Trusts) by NHS England. Suggested revisions to the layout were offered/supported and these are included in the changes submitted to Radar.

A. We are not aware of any Trust that has gone live with LFPSE having the ability to amend the mandated LFPSE questions set by NHSE (if there is evidence of this, please let the Patient Safety team know so they can make contact and follow this up).

A. Additional information can be included on the form but given it’s already lengthy, the option to access supporting information via the hyperlink was the format/tool considered to be most efficient. This can be revisited after the go live dates if we receive adverse feedback from users. Also, it is important to note that “guidance information pop up boxes” will be included in the form with useful information to support users with any issues.

A. Optional questions only appear if certain criteria have been selected about the incident (i.e. what didn’t go to plan), currently these are not mandatory. We will revisit this post go-live (decide/agree where they are positioned on the form via the Radar User Group).

A.  The form can be ordered differently, but to make sure our testing is completed on 16 May, a cut-off for amendments was required and this proposal was received outside of the timeframe. This can be worked through collectively pre/post go-live to agree the running order.

Testing will take place on the 16 May and go live on 30 May. The proposal is to convene a forum of leads to take stock of the effectiveness and changes (in scope for MCFT to amend such as the running order and minor changes that are not set by NHSE) during the first week of June to agree actions/next steps.

Our priority is testing and the go-live on Radar. Any other changes would impact on our deadlines that have been agreed at Board and NHS Cheshire and Merseyside.
