Learning from Patient Safety Events (LFPSE)

The key impact and changes for your reporting is now available in a user guide. Reporting changes take effect from 1 October and all relevant patient safety incidents reported on Radar will be automatically uploaded to LFPSE rather than the current system of NRLS. 

There are additional questions you will be asked as we go live but we are working on consolidating these into our own incident reporting form by the end of 2023. 

Also essential to note, NHS England have issued updated harm levels for both physical and psychological harm and these (definitions) can also be found in our guide.
