Cause of Death enquiries

As outlined in the inquest policy, please can any requests to the coroner for the Cause of Death (COD) be made through the inquest team via the generic email inquests@merseycare.nhs.uk. If a clinician emails a request for the COD directly to the coroner, this may open up dialogue between them. The inquest team has been experiencing an increase in instances where coroners are approaching the divisions directly for statements and records. The inquest team is happy to make these requests, however, we would ask that this responsibility is allocated to one person in the divisional teams to avoid duplicate requests.
