As a Trust we are experiencing unprecedented challenges and Patient Safety Incident Reporting is of upmost priority to enable us as a Trust to report on our level of incidents and reflect the impact of this on our patients and staff. This includes all levels of incidents including those which result in low harm/no harm. Please can we remind all staff to report incidents using the links below :
