Whilst the Trust is reviewing information and guidance on the Libre glucose monitoring device, we have decided to only use the Trust’s blood glucose/ketone monitoring machines and standard operating procedures (SOPs). Therefore, if a patient’s admitted to our inpatient wards or community caseload and they currently use a Libre device, and require insulin administration or blood sugar monitoring, staff must inform the patient that we will use our equipment to test their blood sugar.

If the patient does not agree and declines to have their blood sugar taken using our equipment, please document this and make sure they have capacity to make the decision by using the shared decision making process.

Documentation needs to clearly state reasons why the patient is declining blood sugar checks. Patients will be required to continue monitoring blood using the Libre machine and share their readings. This will need to be escalated to medical staff for inpatients and also with our community nursing teams, as required.
