    Amendments to Rio Outcome Measure Forms: 29 August 2023

    Colleagues working within community based adult mental health services (including early intervention in psychosis and eating disorder services), dietetics, falls teams and relevant physiotherapy and occupational therapies are advised that the Goal Based Outcome Tool – Adult is now live in Rio. You can find this form in Outcome Measures/Screening Tools within the Outcome Measure folder accessed via the case record menu.

    Dementia friendly door wraps

    Staff from the Mersey Cares charity are visiting projects funded by the former Dragons Den Scheme to understand the impact. They recently visited Fern Ward at Leigh Moss Hospital, older people’s inpatient services where funding has allowed them to install dementia friendly door wraps on bedroom doors.
