Community Services

Our staff deliver community health services to people in their own homes, in locations including health centres and clinics, NHS Walk-in Centres and an older adults intermediate care ward at Aintree University Hospital site.

We are the largest provider of 0 to 19 services across Liverpool and Sefton, working in partnership to improve the health and wellbeing of 160,000 local children and their families.

We also deliver specialist dental health care, therapies, medicines management and nutrition and dietetics service. Our teams work closely with GPs and other health and social care services to ensure patients are cared for with the best support, closer to their home.

Our community nurses enable people to be cared for in their own community and remain as independent as possible, rather than having to go to hospital. Many of these services are delivered out of hours, 24 hours, seven days a week.

Every year our staff deliver care to thousands of patients and at the same they will be talking to their families, carers and their communities. These hundreds of thousands of interactions mean that our staff are best placed to understand patients’ holistic health and social needs, including education, prevention and self care.

    Relocation of Liverpool Community Equipment Service

    4 September 2023

    The Community Equipment Service is relocating to new premises at Mersey Reach, Unit 5, Dunnings Bridge Road, Liverpool L30 6UZ. Work is underway to carry out a ‘lift and shift’ of all equipment, storage shelves and office furniture from the current location at Graylaw.

    Amendments to Rio Outcome Measure Forms: 29 August 2023

    Colleagues working within community based adult mental health services (including early intervention in psychosis and eating disorder services), dietetics, falls teams and relevant physiotherapy and occupational therapies are advised that the Goal Based Outcome Tool – Adult is now live in Rio. You can find this form in Outcome Measures/Screening Tools within the Outcome Measure folder accessed via the case record menu.

    Carole Westhead is retiring

    Carole Westhead, Operational Administration Manager, is retiring at the end of August, taking some well earned quality time to spend with her family and friends after 40 years NHS service and we would like to wish her all the very best for her future.

    Ann Gill, Administrator for Telehealth is retiring at the end of the month. Ann is the longest serving member of the admin team, having been with the team since 2018. Her team will miss her but wish her all the best in her retirement!
