Mersey Care as an NHS Foundation Trust is committed to continually assessing and managing the risk of harm to others in conjunction with partner agencies. Risk is being coordinated through a robust structured risk management process that incorporates MDT reviews, H-RAMM and MAPPA. This policy will support those people who present with needs that sit outside of CPA and MAPPA and require a more formal approach to risk management.
