MMP53 Medicines Management in the Integrated Community Reablement & Assessment Service (ICRAS)
This procedure provides guidance on processes relating to medicines that staff working within the ICRAS team in Liverpool & South Sefton must adhere, including changing regular medicines for patient’s admitted to ICRAS; prompting and observing patients to take their medicines by Health Care Assistants / Health Practitioner Assistants; administration of prescribed topical medication/preparations by Trained Health Care Assistants / Health Practitioner Assistants; transfer of Care From ICRAS to the Care Package Provider where medication administration/prompting is in place; medicines reconciliation for patients referred to ICRAS; and process for referring to ICRAS Medicines Management Team.
