The Trust-wide policy on Reporting, Management, Review and Learning from incidents (SA03) has recently been updated. This is to strengthen the content with regard to the just and learning approach taken to review and learning from incidents. It also gives guidance to Divisional teams around roles and responsibilities for incident management. The new policy can be found on YourSpace.
