Policy is issued as a framework to support Information Governance and effective health records management.

The Francis Report outlined several standards that should be met when a person who has been receiving in-patient care dies which relate to the certification of death and the assessment of whether all could have been done to either prevent the death or improve on the care of that person during the terminal stages of their life. The same standards also apply to Service Users in the community
