We have identified nine transformation priorities (and work streams) to support the delivery of changes which include introducing new systems, processes and more collaborative ways of working to make our care more effective and efficient, to developing new models that better meet the needs of our service users and patients who require urgent same day or planned care services.
A series of workshops with operational and clinical staff have taken place with a focus on the nine work streams and have supported the development of a co-produced plan for year one and what changes we will deliver.
Progress has been made since we started the work in September 2023. All programmes of work have now reached the delivery phase. In the next few weeks Whole Person will also have progressed to this critical phase. Key achievements have already been made but change and transformation takes time to progress and embed. The work continues.
