Across Cheshire and Merseyside, work is underway to deliver a single, region-wide shared care record platform called the Cheshire and Merseyside Care Record (CM Care Record).
Mersey Care colleagues can access the CM Care Record through Rio and EMIS Electronic Patient Record (EPR) systems.
As part of the ongoing development of the CM Care Record, some important changes have been made to the eXchange/Share2Care care record, as outlined below.
Changes to eXchange/Share2Care
The Primary Care button has been renamed EPaCCS. The GP record will still be available under this button until 31 March 2026, after this point users will need to access primary care (GP) information via the CM Care Record.
The CM Care Record is accessible directly from EMIS - listed as the 'Graphnet Portal’ and from Rio - listed as the 'Cheshire & Merseyside Care Record (CMCR)'.

Please note: Clinical correspondence and end of life (EPaCCS) information can still be accessed from eXchange/Share2Care.
Accessing primary care (GP) data through the CM Care Record
Across Cheshire and Merseyside, GP practices use EMIS, TPP and Medicus as their clinical systems. The CM Care Record receives nightly data feeds from EMIS and TPP. Alongside this, all these systems are live with GP Connect which is a national service that provides an England-wide view of real-time primary care data. GP Connect can be accessed from the CM Care Record via the link pictured below.

Please note: If your patient’s GP practice is outside the Cheshire and Merseyside region, the nightly primary care data feed will not appear in the CM Care Record. In these instances, please select the GP Connect real‑time information tile (pictured above) to view the England‑wide GP record.
Using the CM Care Record
The first time you access the CM Care Record you will be asked to complete a new user profile form (pictured below). Please select your role by typing in the form field to display the available options. If you cannot locate your specific role, please select the nearest match. This information is used for audit and reporting purposes only.
