This procedure describes the processes that should be followed for the initiation and maintenance of anticoagulant therapy for a patient during their inpatient admission. It provides guidance on Low Molecular Weight Heparin, vitamin K antagonists and Direct Oral Anticoagulation treatments in relation to assessing eligibility, contraindications and cautions; consent and counselling; prescribing and care planning; administration and monitoring; discharge planning and obtaining supplies.

This policy supports the requirements of the NHS England Patient Safety Incident Response Framework (PSIRF) and sets out how Mersey Care NHS Foundation Trust (the Trust) will approach the development and maintenance of effective systems and processes for responding to patient safety incidents and issues for the purpose of learning and improving patient safety.

The effective management of incidents is an integral part of the way the Trust meets its duty to minimise the risk to its service users, carers, staff and visitors, with the aim of maintaining their health and safety. There have been repeated instances when information sharing could have prevented a child being seriously harmed. When Serious Case Reviews have taken place it has been established that information about historical safeguarding concerns with regard to other children related to adults living with the child, had not been shared between agencies. A recurrent theme within the analysis of Serious Case Reviews is that it is vital for practitioners to share information to safeguard and protect children (Brandon et al, 2009)It is essential that Child Health Records of newborn children must include full details of all siblings, step siblings and other children up to the age of 18 and any adult who may live in the family home of the newborn.

Early Intervention Teams (EIT) have now been established nationwide due to the recognition that early effective treatment in psychosis contributes to better recovery rates for both service users and their families (National Institute for Health and Care Excellence; NICE, 2014; NICE, 2013; NICE 2016b; NICE, 2018). This document brings together and updates the existing operational policies for the Wigan & Leigh, St Helens & Knowsley and Warrington & Halton Early Intervention in Psychosis (EIP) Teams.

The Discharge Medicines Service enables NHS Trusts to send referrals direct to the nominated community pharmacy of patients who are eligible and would benefit from extra guidance around newly prescribed medicines, including a copy of the patient's discharge prescription, aiming to reduce re-admissions to hospital. The automated referral process and eligibility are determined in this SOP.

This policy and procedure will provide guidance to community staff in the actions that should be taken when they identify that service users have access to weapons.
