MMP11 Discharge Medicines Services SOP
The Discharge Medicines Service enables NHS Trusts to send referrals direct to the nominated community pharmacy of patients who are eligible and would benefit from extra guidance around newly prescribed medicines, including a copy of the patient's discharge prescription, aiming to reduce re-admissions to hospital. The automated referral process and eligibility are determined in this SOP.
