It has been identified that clinical information is not being stored within the patient record and is being stored / filed separately.
The Electronic Patient Record (EPR) is the primary record and information must be scanned/uploaded into the patient record and not kept separate.
A fragmented patient record is a patient’s clinical record that has information stored / filed separate from the primary record (Electronic Patient Record) and is a risk to patient safety. The Trust must have the right information about the right patient in the right place at the right time.
