We have an exciting secondment opportunity for an experienced leader to support the Cheshire Liaison and Diversion Service and Criminal Justice Team as Clinical Team Manager for a period of six months whilst we recruit to the substantive post.

If you'd like to express interest in this role, you will need the full support of your current team manager, with a view of commencing in post as soon as possible (ideally the beginning of October). The post would require vetting clearance from Cheshire Police but this can be completed after you have started in post. Experience in this area is desirable but not essential, however we would require leadership experience.

Liaison and Diversion Team:

Covering Warrington and Halton area of the Cheshire Contract.

Working in partnership MITIE care and custody and Cheshire and Wirral NHS Foundation Trust.

4 Practitioners covering Warrington court, Runcorn custody and community, 4 Facilitators (support custody with vulnerability screenings) 1 community navigator (support people to access health and social services and provide emotional support through criminal justice process)

  • Aims to reduce health inequalities
  • Improves access to healthcare and support services for vulnerable individuals who come into contact with the Criminal Justice system
  • Liaison with healthcare and support services to deliver a coordinated response, ensuring that the needs of individuals are met
  • Diversion of individuals, where appropriate, out of the youth and criminal justice systems into health, social care, education and training, or other supportive services
  • Identifies those individuals with communication/participation difficulties and where appropriate recommends measures to facilitate effective participation
  • Aims to deliver efficiencies within the youth and criminal justice systems
  • Aims to reduce re-offending and/or escalation of offending behaviours

 

Criminal Justice Team:

Covering boroughs of St Helens, Knowsley, Warrington and Halton.1 Practitioner allocated to each area. 1 x admin support across all areas.

  • Attend referral meetings at designated local prisons: HMP Liverpool and HMP Altcourse liaise with other prisons as required
  • Case manage remand prisoners with identified MH needs to ensure continuity of care
  • Improve identification, screening and assessment of mental vulnerability of adults in Criminal Justice settings
  • Facilitate assessments in accordance with CPA, MHA, MCA, Equality Act 2010 and local and national service pathways
  • Support access to appropriate health and social care for individuals in contact with the criminal justice system
  • Support and contribute to the management of risk to the public (MAPPA)
  • Offer Advice, Consultation and Liaison between the relevant Health, Social Care and Criminal Justice agencies
  • Contribute to Probation Pre-sentence Reports to facilitate a holistic picture of risk and need thus enabling informed and appropriate outcomes
