On 23 February, the executive summary of the Independent investigation into the treatment and care of Ben (pseudonym) was published by NHS England and is available to read on our website in the independent reports section. Ben was convicted of manslaughter in January 2019. At the time of the homicide Ben was receiving care and treatment provided by our Trust.
NHS England have also produced a Lessons Learned Bulletin, with actions for individual practice, governance and system learning. Please take some time out to read the bulletin and take any actions as appropriate.
