When the new GlucoRx HCT blood glucose monitors are delivered to your team, an envelope addressed to Eddie Whelan, Medical Devices Dept. will be provided.  

Please follow these instructions when returning all other blood glucose meters (BGMs) including the Roche Accu-Chek Performa’s and the Performa Nano’s:

  • E-mail medicaldevices@merseycare.nhs.uk and request a ‘Medical Devices Depts Decontamination Certificate for BGM
  • Clean/ decontaminate all BGM being returned
  • Remove all batteries from all BGM
  • Place all cleaned BGM & the removed batteries in the addressed envelope (for teams with multiple BGMs, please use a cardboard box)
  • If using internal mail:
    • Place the decontamination certificate in the envelope/box, seal the envelope/ box and post
  • If you are arranging a collection:
    • Attach the completed decontamination certificate to the envelope (or place in the box)
    • E-mail medicaldevices@merseycare.nhs.uk and arrange for the BGMs to be collected at a convenient date, from a suitable collection point.

Please note:

It is a legal requirement to clean or decontaminate all previously used medical devices prior to them being posted or transported. A completed decontamination certificate must be attached to all posted or transported used medical devices.

If you have any unopened ‘Roche Accu-Chek Test Strips’, ‘QC solutions’ or ‘QC Log Books’ that you no longer need, we will be happy to collect these and redistribute them amongst the teams that are still using the Roche BGMs.
