Our new carers passport is now live and available on our inpatient wards. The passport is a simple tool which identifies someone as being in a caring role for one of the hospital's patients, involving them more fully in the patient's care, and connecting them with further support.
A carers passport can benefit carers, patient and staff by:
- Ensuring patient’s individual needs and preferences are understood.
- Ensuring carers have the flexibility and opportunity to be present with their loved one, to provide emotional support and reassurance when they are frightened and anxious
- Having carers present may help to reduce agitation and distress for the patient
- Supporting communication, helping the patient to understand and make sense of what is happening.
- Encouraging compliance with treatment regimes
- Allowing the carer to provide support and encouragement, with eating and drinking.
- Including the carer in discussions around care and discharge plans
- Making the offer from the Hospital/ward explicit.
