Following the merge of the two versions of RiO, there is an ongoing piece of work to review existing forms to ensure that they are fit for purpose. Users may notice some slight changes to the following outcome measures/screening tools:
DIALOG
The “Reason for Non-Completion” drop down list option “Service User Refused to Complete” has been revised to “Client Declined to Complete".
Severity of alcohol dependence questionnaire
The form has been updated to include the NHS number for the client. This should pull through automatically from the client registration.
