The St Helens and Knowsley Community Specialist Palliative Care Team is currently working at reduced capacity and this is expected to last until September.
How we are prioritising referrals
So that the team can respond to those with the greatest need of specialist support, referrals will be triaged and prioritised against the criteria below:
- patient is actively dying and symptoms are not controlled
- patient has complex uncontrolled symptoms that are not managed by the current plan implemented by the primary care team
- patient is being referred from the acute setting and has complex and uncontrolled symptoms that will require specialist assessment and management
- request is for a medical assessment and the patient is not able to attend an outpatient clinic
Your support
If you need to make a referral to the team, please consider if the face-to-face outpatient services available at Willowbrook Hospice and Whiston Hospital (including psychological support services via Outreach Services based at Willowbrook Hospice) would be able to support your patient. This can be indicated on the joint referral form.
