Continuing Health Care Assessments (CHC) including Fast tracks, Decision Support Tools, and CHC reviews has historically been commissioned in a variety of ways across the North Mersey area. Mersey Care has merged its function in Liverpool and Sefton into one team, while provision in Knowsley has sat with the District Nurse Liaison team.
Regionally there has been review of the patient journey across Cheshire and Merseyside (described as an end-to-end review) and there has been a decision to bring all the elements of this service in the region together, provided by the Cheshire and Merseyside Integrated Care Board (ICB).
Notice has been served to Mersey Care on the contract for the provision of CHC assessments and reviews. Staff in these services will TUPE into the Integrated Care Board (ICB) along with other services across Cheshire and Merseyside and the ICB will be the provider of these services from 1 April 2024.
