To support infection prevention and control, a change has been introduced within our clinical systems to enhance our patient admission process.

When a patient is admitted to our care, you will now need to complete a patient infection prevention and control screening questionnaire.

You will be prompted to complete this questionnaire within the clinical system you are using as part of the patient admission process.

The questionnaire consists of six questions that capture data on risks of infection including:

  1. Diarrhoea and/or vomiting
  2. Unexplained rash
  3. Fever
  4. Respiratory symptoms
  5. History of Multi-Drug Resistant Organisms (MDRO) such as MRSA
  6. Isolation for any other reasons

Capturing this information is important as it will help us identify and manage infection risk, and will enable us to signpost you to the appropriate guidance and support.

The form can be located within ‘Case Record Summary’ within the ‘Inpatients’ tab and then ‘Inpatient Forms’. The document is called ‘IPC Admission High Risk Criteria Questions’.
