During July 2022 the two RiO clinical records systems used at the Trust will merge to create one Trust wide RiO system. In readiness for this, we wanted to make you aware of some changes to the Safeguarding documentation expectations within RiO.

The first change is the name of the forms you are expected to complete to record safeguarding children and adult concerns. There will be some minor changes to the form itself, however the legacy name of Safeguarding Communication Form will change to Initial Safeguarding Record.

The newly named Initial Safeguarding Record will continue to be used to document any safeguarding concerns, record multiagency referrals made and capture internal actions taken by staff; at the point of a concern being raised or a need being identified.

Subsequent safeguarding activity, communication or information relating to that Initial Safeguarding Record, is then logged by documenting it in an easy to complete Safeguarding Activity Update. Both forms will remain accessible through the Safeguarding tab in every clinical record.

The second change is that you will be able to attach an Initial Safeguarding Record to the RiO referral open to your own team and there is no need to complete a referral to the Safeguarding Child or Adult Team.

Safeguarding advice, support and supervision regarding clinical cases or record keeping process remains as Monday to Friday 9am to 5pm by calling 0151 244 3331 (Safeguarding Children) and 0151 244 4588 (Safeguarding Adults).
