October's Employee of the Month winner is Emily Courtney, Churchtown District Nurses nomnated by Ashlie Manby
'Emily has shown immense respect for a member of the public during a scheduled home visit to a patient on the District Nurse caseload. Emily identified that a member of the public needed help prior to visiting a patient. The gentleman could not walk properly and could not make his way into his property. Emily and the student nurse assisted him back to his home, where they found his living conditions were extremely poor. He struggled to communicate with Emily, Emily found notes on his windows that stated he needed help. He wrote down he hadn’t eaten for 5 days and could not get out to the bank for money. Emily purchased food for him and contacted his Next of Kin (NOK). His NOK had been unable to contact him for 2 years, he had no registered GP. The gentleman refused an ambulance, however the NOK persuaded him. The NOK could not thank Emily enough for the kindness and compassion she had shown for the gentleman. The gentleman was diagnosed with likely MND during that hospital admission.
This gentleman was a member of the public that Emily treated with kindness, respect and compassion when other people thought he was under the influence of alcohol. He will now get the care he needs for MND and has been reconnected with his NOK. The gentleman was living in extremely poor conditions, he had not eaten for days and was in severe ill health. The NOK had been trying to contact him for over 2 years and couldn’t thank Emily enough for taking the time to help this gentleman. The Team at Churchtown DNs are extremely proud of Emily. The service has been extremely busy; however, Emily took the time to ensure this gentleman was safe. I believe that his health outcomes would have been very poor if it wasn’t for Emily’s kindness that day".
October's Team of the Month winner is Childwall District Nurses nominated by Lyn Cane
"Three years ago, this team was on the risk register due to shear amount of concerns escalated, patient harm and. Over the last three years the staff have worked above and beyond on a daily basis. The Team have gone from poor communication affecting patient care to showing passion in their work wanting to improve the team’s dynamics and patient care. Each and every team member have all played an important role in the development of this team and recently receiving a good plus in our overall rating in district nursing. This team have thrived over last few years and continue to do so. When considering the pressures over the last 18 months and devastation this team has seen they continued to work above and beyond. Each staff member ensured they played their role in ensuring the patients received high standards of care. All team members have pushed themselves to excelling their roll and learn the skills required to continue to improve the care they provide and deserve to be recognised for this.
The team are no longer on the risk register. They work to a high standard all the time, which has been recognised numerous times by the excellent documentation at 72-hour review Patients give positive feedback all the time We have not had any investigations for a long time due to omissions where harm has occurred. Staff now have an excellent rapport with one another and are excellent support for one another Such a positive team to work in although extremely busy staff member will still work above and beyond to get the positive results needed. Patient’s care is to an excellent standard which reflects from feedback from senior managers and other health profession teams Team received an excellent report recently from QRV and hope for a crepitation next year. This team has gone from strength to strength and each team member has played a crucial role and should be proud of themselves. Well done Childwall DN's"
