NCEPOD are conducting a national study to explore the barriers and facilitators in the process of the transition of young people with complex chronic conditions from child to adult health services. They are inviting clinical staff to complete a survey. To find out more about the study, visit the NCEPOD website.

Who should complete this survey?

Please complete this survey if you are or could be involved in the care of young people (between 13 -25 years of age) with the following complex chronic conditions: Epilepsy, Sickle cell, Juvenile idiopathic arthritis, Cerebral palsy, Spina bifida, Muscular dystrophy, Post-transplant patients (heart, liver or kidney), Brain Tumour (Medulloblastoma), Autism spectrum disorder, Rett syndrome, Type 2 diabetes in obese young people, Chronic kidney disease.

NCEPOD are collecting data from health and social care providers across a range settings, including primary care, community care (including school nursing and hospices), physical healthcare, mental healthcare, commissioning and social care.
