Medical Appraisal and Revalidation Policy NWBH The aim of this policy is to ensure that all licensed medical practitioners (doctors) with a prescribed connection to Mid Mersey Division (formerly North West Boroughs Healthcare NHS Foundation Trust)undergo a high quality and consistent form of annual medical appraisal.

The aim of the process outlined in this document is that all members of the Board of Directors, the Executive Team and their Direct Reports have been subject to relevant the Fit and Proper Persons Test on an annual basis.

Around 40% of people who have been diagnosed with psychosis have also misused a substance at some point in their lifetime. This is at least double the rate seen in the general population. This policy aims to provide guidance to practitioners in Mid Mersey Division to assist service users with complex needs in a way that is effective, respectful to the individual’s human rights and within the law.

The Discharge Medicines Service enables NHS Trusts to send referrals direct to the nominated community pharmacy of patients who are eligible and would benefit from extra guidance around newly prescribed medicines, including a copy of the patient's discharge prescription, aiming to reduce re-admissions to hospital. The automated referral process and eligibility are determined in this SOP.
